🏕️ Act I — Rogue Encampment Kill Andariel
+Skill Points
Complete Den of Evil (Blood Moor) — talk to Akara for your reward
Free skill point. The cave is right outside camp. This also grants a free respec — one per difficulty, never waste it early.
Pro Tip
Save your Respec from Akara — don't use it yet
You get one free respec per difficulty from Den of Evil. Save it for when you hit a wall in Nightmare or Hell and need to rebuild your character.
Skip This
Skip Blood Raven (Sisters' Burial Grounds)
Optional quest on all difficulties. Reward is a random magic ring — rarely useful. Not worth the detour unless you're passing by anyway.
Pro Tip
Rescue Deckard Cain (Tristram portal in Stony Field)
Optional — but absolutely worth doing. To rescue Cain: find the Cairn Stones in Stony Field and click them in the correct order shown on the ground nearby. This opens a portal to Tristram. Enter, kill the monsters, and click Cain to free him — he teleports back to the Rogue Encampment automatically. If you skip it and advance to Act II, the Rogues rescue him instead and he charges 100 gold per identification from then on. Rescuing him makes all identifications free permanently.
Pro Tip
Farm the Countess (Forgotten Tower, Black Marsh) for runes
The Countess drops 1–6 runes guaranteed per kill. Run her until you have Ral, Ort, Tal, Amn, Thul for Spirit and Ancient's Pledge. Her tower has 5 floors — keep the wall on your left at all times to find the stairs.
Skip This
Skip the Horadric Malus quest (The Smith, Barracks)
The reward is a socketed Normal helm or armor — vendor quality at any stage of the game. Skip it on all difficulties. The Smith is also one of the more dangerous early enemies.
Pro Tip
Activate Waypoints: Cold Plains, Stony Field, Outer Cloister, Catacombs L2
Only grab waypoints on your critical path. Skip Burial Grounds, Black Marsh, Tamoe Highland — you won't need to return.
Pro Tip
Hire an Act 1 Rogue mercenary (Cold Arrow or Fire Arrow variant)
The Rogue is a solid early companion. Choose Cold Arrow for crowd control or Fire Arrow if you need to avoid freezing corpses (e.g. for Corpse Explosion builds). You'll likely replace her with an Act 2 merc in Nightmare.
Mandatory
Kill Andariel (Catacombs Level 4) to unlock Act II
Stock up on Antidote Potions before the fight — her poison damage is brutal. Drink 3–4 right before engaging. She is weak to fire.
🏛️ Act II — Lut Gholein Kill Duriel
+Skill Points
Kill Radament in the Sewers (Level 3) — pick up the Book of Skill he drops
Free skill point. The sewers are in town — go down two levels. Also reduces vendor prices in Act 2.
Pro Tip
Acquire the Horadric Cube from the Halls of the Dead (Dry Hills)
Essential item that you use forever. Located in the Halls of the Dead Level 3 (Dry Hills → Halls of the Dead → go down to L3). The chest containing it is guarded by Bloodwitch the Wild, a Super Unique. The Cube is in a golden chest near her — hard to miss. No waypoint inside, so take the Far Oasis or Lost City waypoint and navigate back.
Pro Tip
Hire an Act 2 Desert Mercenary and keep him alive
Act 2 mercs are the best in the game and most builds use them all the way to endgame. The aura you hire matters — in Normal only three auras are available (Blessed Aim, Defiance, Prayer). In Nightmare and Hell all six are available, including Might and Holy Freeze, which are the endgame picks. Hire whoever keeps you alive in Normal; respec your merc choice when you reach Nightmare Greiz. See the Mercenary Guide for full aura and gear recommendations.

Skip the Arcane Sanctuary secret area exploration (just find the portal)
Mandatory to progress. To reach it: from Lut Gholein, enter the Palace (in the east part of town), go through Palace Cellar L1 → L2 → L3, then talk to Jerhyn at the bottom to access Harem L1 → L2, which leads into the Arcane Sanctuary. Once inside, find the glowing portal to Tal Rasha's Tomb — note the symbol on the wall near the portal, you'll need it to identify the correct tomb in the Canyon of the Magi. Don't bother clearing the whole Sanctuary.
Mandatory
Combine Staff of Kings + Viper Amulet in Horadric Cube → Horadric Staff, then kill Duriel
Once you have both pieces, transmute them in the Cube to create the Horadric Staff. Then head to the Canyon of the Magi (accessible from the Arcane Sanctuary portal or by foot through the Palace). There are seven tombs here — six are false, one is Tal Rasha's True Tomb. The True Tomb is identified by a symbol on your mini-map matching the symbol shown on the wall in the Arcane Sanctuary. Enter the True Tomb and find the orifice (a tall receptacle on the wall) — place the Horadric Staff into it to open the chamber to Duriel. ⚠️ Duriel hits like a truck and his freeze is brutal. Open a Town Portal before entering his chamber, stack Thawing Potions, and be ready to flee through the portal immediately if needed.

🪵
Staff of Kings
Far Oasis WPMaggot LairLevel 3 Navigation: take the right-hand path on L1, then left-hand path on L2 for the fastest route down. No waypoint inside the Lair. Coldworm is immobile and has no attacks — the chest is to her left. ⚠️ Don't stand on her when she dies — her death spray poisons to 1 HP instantly
📿
Viper Amulet
Lost City WPhead East → up stairs → NW → circle the 3-door structureValley of Snakes → Claw Viper Temple L2 On L1, travel left for the fastest route to the L2 stairs. Smash the altar to drop the amulet — don't look for it on Fangskin. ⚠️ Teleport and Town Portal are disabled on L2 — open a portal at the top of the stairs before going down
Staff of Kings + Viper Amulet → Horadric Cube → Transmute Horadric Staff
🌴 Act III — Kurast Kill Mephisto
+Skill Points
Complete The Golden Bird quest — drink the Jade Figurine potion from Alkor
+20 permanent max life per difficulty (+60 total). The quest has four steps: (1) Kill the Super Unique Ku Y'leh in the Flayer Jungle — he drops a Jade Figurine. (2) Bring the Figurine to Meshif (the boat captain in Kurast Docks) who gives you a Gold Bird in exchange. (3) Bring the Gold Bird to Alkor (the alchemist in Kurast Bazaar) who gives you a Potion of Life. (4) Drink the Potion of Life to permanently gain +20 max life.
+Skill Points
Complete Lam Esen's Tome — bring the Tome to Alkor
+5 free stat points per difficulty — +15 total across all three runs. To complete it: find Alkor in Kurast Bazaar and talk to him to get the quest. Then find the Black Book (Lam Esen's Tome) inside the Ruined Temple or Disused Fane — one of five small dungeons scattered across Lower Kurast, Kurast Bazaar, or Upper Kurast. The dungeon containing the Tome is random each game, so check each one until you find it. Bring the Tome back to Alkor to claim your reward.
Skip This
Skip The Gidbinn and The Blade of the Old Religion quests
Rewards are a random rare ring/amulet with poor odds of being useful. Not worth the effort on any difficulty.
Mandatory
Collect Khalim's organs → transmute into Khalim's Will → smash the Compelling Orb
Required to enter the Durance of Hate where Mephisto is. Collect all four pieces, transmute them in the Horadric Cube, then equip Khalim's Will and attack the Compelling Orb.
Where to find the pieces
👁️
Khalim's Eye
Spider ForestSpider Cavern Golden chest guarded by Sszark the Burning. ⚠️ There are two caves in Spider Forest — the other is the Arachnid Lair and has no Eye. If you don't see a golden chest you're in the wrong one
🧠
Khalim's Brain
Flayer JungleFlayer Dungeon L3 The dungeon entrance is always inside the small Flayer village. Go to L3 and find the golden chest guarded by Witch Doctor Endugu. ⚠️ The Swampy Pit looks identical but is the wrong dungeon
❤️
Khalim's Heart
Kurast Bazaar or Upper KurastSewers L1 → L2 L1 is large and random — look for a lever on your automap (red dot) guarded by Icehawk Riftwing (teleporting bat, cold enchanted). Pull it to open the trapdoor. L2 is fixed layout — small room, Heart is in the chest at the northeast end
⛏️
Khalim's Flail
Upper Kurast → Kurast CausewayTravincal Dropped by Ismail Vilehand — the first of the three Super Unique Council members you kill. The Compelling Orb is on the left side of the entrance to the Guardian Tower. Equip Khalim's Will and strike it several times to shatter it
Eye + Brain + Heart + Flail → Horadric Cube → Transmute Khalim's Will · Equip and attack the Compelling Orb
Pro Tip
Use the Moat Trick on Mephisto — a safe strategy every new player should know
In Durance of Hate Level 3, Mephisto's arena has a shallow moat running through the room. Mephisto's AI won't let him cross it — but your ranged attacks and spells can reach him across it just fine. Position yourself on the far side of the moat and attack freely. Melee characters can't use this trick.
Mandatory
Kill Mephisto — then take the red portal to Act IV
Use the Moat Trick if you can. Farm Nightmare and Hell Mephisto repeatedly later — he has an excellent loot table and is one of the best gear farms in the game.
🔥 Act IV — Pandemonium Fortress Kill Diablo
+Skill Points
Kill Izual (Plains of Despair) — talk to Tyrael for your reward
Two free skill points. Izual is tanky but not dangerous. Take the City of the Damned waypoint and head into the Plains of Despair from there — there is no waypoint in the Plains itself.
Skip This
Do Hell's Forge on Nightmare and Hell — skip it on Normal
Hell's Forge guarantees a rune drop that scales with difficulty — poor on Normal, up to Gul on Nightmare, Ist–Gul range on Hell. To start it: you must pick up Mephisto's Soulstone from the ground in Durance of Hate L3 after killing Mephisto in Act III — it doesn't go to your inventory automatically. Carry it into Act IV, then find Hephasto the Armorer (near the River of Flame waypoint) and kill him for the Hellforge Hammer. Take both to the Hellforge (a large brazier in the River of Flame area) and use the hammer on it to destroy the Soulstone and collect your rune reward.
Mandatory
Activate all 5 seals in Chaos Sanctuary to spawn and kill Diablo
Five seals across three wings (two left, two right, one center). Each wing spawns a Seal Boss when both its seals are active. All three Seal Bosses must die before Diablo appears. ⚠️ Oblivion Knights cast Iron Maiden — purple swirls on your character means stop attacking immediately or you'll kill yourself.
⛰️ Act V — Harrogath Kill Baal
Gear Priority+Skill Points
Complete Siege on Harrogath (kill Shenk) — get your free socket from Larzuk. Then rescue the Barbarians from Qual-Kehk for a rune reward
Larzuk adds the maximum sockets to any unsocketed item — save it for a good helm or weapon. Qual-Kehk's rune reward is random but often useful for Ancient's Pledge (Ral+Ort+Tal in a 3-socket shield = +35–48% all resistances). Rune order matters.
+Skill Points
Complete Prison of Ice — free Anya from the Frozen River, use Malah's scroll
+10 permanent all resistances per difficulty (+30 total). To complete it: find the Frozen River (accessible from Crystalline Passage, Act V) and locate Anya — she's trapped in a frozen pond near the end of the area. Click her portrait to free her. She returns to Harrogath automatically. Then talk to Malah (the healer in Harrogath) who gives you a scroll — use it on yourself to gain the permanent resistance bonus. ⚠️ Nihlathak in Halls of Vaught has Anya's location quest tied to this — you'll need to access the area through Crystalline Passage.
Skip This
Skip Betrayal of Harrogath (Nihlathak) on Normal — consider it on Nightmare and Hell
The quest reward personalizes an item with your character name — purely cosmetic and not worth pursuing. However, on Nightmare and Hell, Nihlathak himself drops the Key of Destruction, one of three keys needed for the Pandemonium Event (Uber content). Once you are farming endgame content, Nihlathak runs become a regular key farm.
Mandatory
Complete Rite of Passage — defeat the Ancients atop Arreat Summit
Required to access Worldstone Keep. To reach them: from Harrogath, progress through Bloody Foothills → Frigid Highlands → Arreat Plateau → Crystalline Passage → Glacial Trail → Frozen Tundra → The Ancients' Way → Arreat Summit. At the Summit, click the Altar of the Ancients (the large stone slab) to begin the fight. All three Ancients must be killed — they have random modifiers each game. If the combination is brutal (especially Cursed + Extra Strong), quit and rejoin the game to reroll the modifiers.
Pro Tip
Target Level 25–30 before killing Baal
Entering Nightmare underlevelled makes the resistance penalty much harder to survive. Baal runs are the most efficient way to level — join multiplayer "Baal run" games for speed. Similarly, aim for Level 60–70 before transitioning to Hell.
Mandatory
Kill Baal (Throne of Destruction / Worldstone Chamber) — Nightmare unlocked!
From the Worldstone Keep Level 2 waypoint, hug the right wall to find the stairs down to Level 3. Level 3 layout is fixed once you learn it.
